    Directorship situation

Long time follower here and have a what I feel like is a strange situation at my workplace. I'm currently the director at my local EM shop. A new doc came in he is EM trained, and seems competent. This guy has been here a little over a month and has become involved in decision making that I...

    All-Star Chief Complaints

OMG are you in Texas? I started telling my Fibro pts the stage of their illness a couple years back. Once they tell me they have fibro I pull up a chair and act very concerned and let them go through all their symptoms. When they finish I'll say hmmm that sounds like stage V Fibro, you better...

    ER Bashing

It just feels like a slap in the face. Especially since they have complemented us multiple times on fx reductions that followed up with them.

    ER Bashing

So I work in a small town of about 36k people at a level 3 trauma facility. I'm one of very few BE/EM docs in the region. The local Ortho group recently decided to open up a Ortho Urgent care. Thats fine and dandy but they have commercials on TV/Radio bashing the local ERs and the Docs. They...

Everything is Sepsis! (when you're part of CMS)

My facility is already doing this. Every time I order a blood culture on a kid they add a lactate. I recently had a nurse wanting to order labs and a lactate on someone who sprained their ankle b/c their vitals met SIRS criteria.

    Investing in Freestanding ERs

There seems to be this mad rush lately to build freestanding ERs. Does anyone have any first hand experience with these from an investment perspective? I'm conisdering cutting back on my regular shifts and buying into one of these.

Tort Reform Doesn't Curb Defensive Medicine

Why did the family keep a brain dead for 5 years? Was the family brain dead? Who did they think was going to pay for the cost? It's ridiculous that they could file a suit, they had a choice to pull the plug. Tort reform does work, but until the standard of care is practiced in every state...
