    Floater Gigs

I don't have anyone screening my direct messages. Over the long weekend I didn't look at a darn thing and came in this morning to literally two messages. One of them was thanking me for having done something else. Another was just confirming an appointment reschedule. Outpatient =!= huge panel...

    Floater Gigs

The problem here is your agency and its institutional culture, not the fact that the job is outpatient.

    Ever ran into a demon?

The Church actually makes it incredibly difficult to arrange for an exorcism and also requires the person receiving it be evaluated to assess whether their behavior might instead be due to mental illness. They can't get rid of it as a sacrament but it is very heavily downplayed. I wonder what...

A question about Psych RXP laws? Collaboration agreements?

It's not the four years of med school as much as the years of residency. I agree we have not done a good job of producing outcome studies clearly showing a population-level difference between outcomes with MDs v. NPs. That does not mean we do not see train wrecks with certain characteristics...

A question about Psych RXP laws? Collaboration agreements?

I am not going to argue that the therapy training in most psychiatry residency programs is adequate by any means, but this is untrue. Training of some kind in at least three therapeutic modalities is necessary for a residency program to be accredited. It is not an elective. Most programs do this...

A question about Psych RXP laws? Collaboration agreements?

Come over to the Psychiatry forum, we complain about the kind of nonsense we see from PCPs and NPs all the time.

How likely is it to make $375-400k in Psych?

Y'all went to school and went through residency for many years in order to acquire a certain lucrative skillset and legal privileges. If you are convinced that your work hours will consistently and reliably generate more income doing something non-clinical, and money is the main goal, what was...

VA Mental Health Provider Venting / Problem-solving / Peer Support Thread

I sometimes share the anecdote with my OSA patients that I have worked with one single solitary person who found CPAP comfortable. He had been a fighter pilot in Viet Nam and the mask reminded him of the glory days. Perhaps worth noting I didn't see him at the VA.

    Commenting on patient's appearance

Chili crab is likewise unavailable in Pennsylvania, it would seem. No Signboard for life. Who am I kidding, though, in S'pore you can throw a brick in a random direction and hit a place selling food so good that it would be in my top three restaurants if it was in my area.

    Commenting on patient's appearance

Where got problem with Singlish lah? Aiyah... EDIT: It is actually a bit more of a proper creole language than an accent per se. The grammatical structure is significantly different than standard English, although the lexicon is English-based. There is also a Singaporean accent that exists but...

    A layman's book on trauma/ptsd?

Richard McNally's Remembering Trauma struck me as being accessible for a reasonably intelligent layperson. EDIT: George Bonanno is pushing his new book, no idea if it's any good but can be pretty sure he's not going to stray into van der Kolkianism

Love working with adult patients' families and providing them with emotional support as well

So much this. I definitely talk about the schizophrenia diagnosis with many FEP folks but initially from a 'you're going to hear people using this word, what's your take on what it means for your life?' kind of perspective. Relentless focus on what they're actually experiencing, genuine...

    Withdrawal and Antipsychotics

These medications were originally developed for use as anesthetic agents so the very first clinical use was in one-off dosing. Neurolepsis never referred to 'anti-psychotic activity' but to the subjective reduction in drive and behavioral output people experienced. If I had to hand wave an...

    Withdrawal and Antipsychotics

Oh yeah, no shade on people who opt against it because there's other options and they don't want to get sued. I just thought it was interesting because in my training there was this drumbeat of "this combo is super dangerous" and it turns out the answer is "....legally"

How likely is it to make $375-400k in Psych?

There is the whole 'your money is going to fund a weird remnant cult from the 70s' thing but they seem reasonably harmless as cults go.
