As a PT, what do you do when you disagree with the doctor's diagnosis?

Just curious, what diagnosis was made by the physician? What diagnosis are you seeing?

    DO Physiatrists/residents: is OMM helpful?

100%. I don't utilize OMT on a daily basis anymore but I still use osteopathic principles and methods daily with respect to educating patients. My reason for not doing OMT is it's physically taxing work. I was introduced to so many MSK and rehabilitation concepts through osteopathic training...

    EMG/NCS and PTs

I wanted to update this thread after reviewing a report by a PT. Very interesting since it's been 7 years and I have a lot more experience now. My personal experience over the years is that the reports from PTs who perform NCS/EMG are general subpar and sometimes flatly just incorrectly done...

    Working with the Military

I'm currently a physician working at an active duty hospital with PTs. There are several civilian PTs that work here. The military also incorporates ATCs into the rehab setting. They often function or are viewed as "PTs." The civilian positions are either government positions (GS) or as a...

    Physical Therapists: Ordering Diagnostic Imaging

@KeikoTanaka I encourage you to keep this idealism because ultimately I think everyone (especially patients) would benefit from collaboration. I think what you are describing is about transparency and ease of data acquisition. Google (of course) is working on something like this already. Very...

    Physical Therapists: Ordering Diagnostic Imaging

I appreciate your optimism! However, this is the exact opposite vision and mission of the APTA. Decentralizing musculoskeletal care away from the medical model where patients need to see physicians first is the overarching theme. PTs want to be first line with respect to musculoskeletal care...

    Physical Therapists: Ordering Diagnostic Imaging

Good catches. However, I suspect for most that these are more the exception than the rule. 8/10 x-rays do not change the course of treatment. In your peds example, your did the right thing by referring them back because of your concern. If you had ordered xrays you still would've referred...

Looking for a specific research article...

There are a lot of these studies across MSK disorders in the ED setting. Check out this article and the references cited: https://www.jospt.org/doi/pdf/10.2519/jospt.2009.2857

Why do patients like chiros more than PTs?

I've worked with PTs, Chiros, ATCs, etc. Chiros definitely have a role and they do help. I think the reason why people like going to them is because of how they explain things. The biomechanical model is very appealing to people. We all use it but it's their main selling point. They can...

    Radiculopathies

Agree with @runfastnow. Pretty much anytime you question someone's ability to return home safely and without assistance you should think about PT and potentially a PM&R consult. Categorically acute or recurrent neurologic injuries and cardiopulmonary patients are common reasons from an adult...

I hear patients say physical therapy doesn't work

    "Sue the doctor." Nice.

    Mid-level Upcreep in PM&R?

The results are useless because of why they are ordered: pain. Personally, I rarely order EMG studies and when I do it's to explain weakness and not pain. If anything, I perform a limited exam to answer certain questions. I digress. Creep is real.
