It really depends. Everyone has their own way of practicing after fellowship. I've seen PMR/sports docs do a mix and still do inpatient stuff. Others work in an ortho clinic as the non-operative person and procedure jockey. I'm FM/sports. Our PM&R sports person does more procedures than I do...
I agree with all of the above. We finalized our rank list the first week of November so any "LOI" at this point will really not influence anything. It's a nice gesture though!
It never hurts. I can tell you during our rank list meeting, amongst ourselves we ask if the applicant ever sent back a thank you letter. It’s a job interview people, please follow proper business etiquette. To us: no thank you letter = no interest.

There are actually some pain fellowships that take FM, and they do interventional spine. FM can now sit for pain boards because of this. Other than that, I suggest going to a sports medicine fellowship that does spine procedures.
