    Fellows Business in Medicine Call

It’s OK, I haven’t taken any of the requests that way. We need feedback on ways to make it better but hope no one gets upset if we end up not adopting their recommendations.

    Fellows Business in Medicine Call

We didn’t record it. I’ll admit now just like I did during the call that I feel quite vulnerable in giving out advice and telling how I do things. It takes a certain amount of courage to put yourself out there because despite what your intentions are, there are inevitably going to be criticisms...

    Fellows Business in Medicine Call

Yeah, it was short notice but I just had to get something going and had been putting it off too long. We touched on all kinds of topics and jumped around a lot. PLanning upcoming discussions with more structure and more experienced people to gain insight from. PMs sent with information.

    Fellows Business in Medicine Call

Following up here, and shout out to Status Sciaticus for nudging me along. Planning to get something together tomorrow night for this. Short notice I know but this is just the first of what likely will be many discussions. I have gotten a lot of interest via private message about this and...

Help me choose between two jobs?

@BobBarker, curious why no on job #1. 4 weeks vacation, percentage of collections pay structure where it seems like overhead is in the 40-45% range and take home pay is 600-700k for 30 patients per day. That doesn’t sound bad at all to me, especially considering it’s such a large group where...

Hospitals are charging unsuspecting patients "facility fees" for routine medical care...

Come work with me, or Bob Barker, or my friend in Southern Oklahoma, or my friend in Nebraska, or my friend just outside of Fort Worth all who are independent private practice folks, and are doing just fine financially. This whole idea that it is financially unsustainable to be in private...

Dummies doing business analysis Boston Scientific

I’m with epidural man, those anchor suture injectors are awesome and do a much better job anchoring than just about all of us do. I pride myself on being the greatest SCS proceduralist I can ever be, do a high volume and work hard to perfect my craft and can admit these things would make me...

    SCS multiple procedure reduction

Correct. Bilateral procedures will reimburse half for the second side, but 63650 is not bilateral. For background and context, I started and run a billing service for private clinics and ASCs specifically in pain management because I felt I could do a better job than what I was paying for. Too...

    SCS multiple procedure reduction

ASCs sometimes bill as Hosptial fyi, not sure if that’s the case or not in your situation. Either way though, I you as the provided would be billing for the professional fee only and it should be as I said, full payment for 63650 x2 and 63685. Some of these hosptial guys can chime in and maybe...

    SCS multiple procedure reduction

The hospital gets paid one lump sum for the procedure, they are not paid per CPT code. The doctor will bill and get paid the professional fee for 63650 x2 plus the 63685. If getting paid by RVU you should get full credit for all codes.

Collections contract review - new attending

Yeah, Very hard to do. Collecting over 1mil year one is hard, unless like you said, the practice gives you a bunch of patients. Even then new grad would have to be pretty fast and be able to see the volume.

Collections contract review - new attending

While I could share my opinion on the questions at hand as others have, which will all vary depending on personal experiences, I will say that my first job was private practice and I got paid a percentage of collections. 33% of collections came to me and there was no bonus structure at all...

    Quick ESI question

I’d do transforaminal with dex and no contrast. No way to confirm good spread but definitely better than telling the patient you have no options.

    Closed loop

Saluda is way more expensive compared to Medtronic/Abbott/Nevro/Boston. We’ve negotiated with them extensively and have mutually decided it’s not going to work. They were 4k more per implant. Yes, they have a C code that allows for additional reimbursement but because of how the additional...
