    I did hear about this. That's a good point. My random opinion would be that a free US MD would probably be preferred over paying for an MD from MSSM for the majority of candidates.

    Doing locums work directly?

    It shouldn't be hard. If you have an accountant and/or lawyer that has mostly dental clients, they can help you set things up easily. Send out letters to every and all offices and doctors within the area you cover offering your locums services. As offices reach out, build your business from there.

    Try to get more letters from orthodontists even if they're not faculty, rather than random faculty members. Highlight your CE, but it doesn't have to be ortho. Taking Invisalign CE as a GP is not particularly impressive IMO so no need to rush to do it and especially don't spend money doing it...

    Owning a practice while specialty residency

    You would need a really strong associate to handle the patients and a super office manager to handle the staff. I know a doc who did it (ortho). He was also more of a business person than a clinical dentist. Is that you? Only you can answer that.
