Idk, seems a bit harder to get a level 3 MDM inpatient from my perspective. Previously simply reviewing old records and summarizing or independently interpreting a study would pretty much get you there, now have to do that plus be on a high risk medication or have a worsening condition / threat...
Agree with stop checking folate as folate deficiency seems to be exceptionally rare based on this article
Also they note - Intake of folate through a meal or ingestion of an oral supplement directly preceding evaluation of serum folate can lead to falsely elevated levels (false negatives)...
DOACs are still off label for LV thrombus apparently, per cardiology at my institution. Said they'd do coumadin for medicolegal reasons (whether that's valid or not) not enough studies on DOACs yet....
