This is all true. Applying to only CA gas progs isn't realistic if your #s are avg. For plan B, you'll have to decide what's more important: doing anesthesia or doing residency in Cali.

What is the best fellowship path?

Make sure you understand the job market before you choose. Do you even need a f'ship for your intended mkt? Also what are the practice models (ie for ICU, maybe the unit at your program is closed with 24/7 intensivist coverage, but where you want to work the units are open with only daytime...

This is the extent of the conversation in PP. Only comes up with new surgeons.

    Ventilation - pressure support

VCV = delivers constant flow, until volume target hit. Pressure is dependent variable (depends on compliance, etc.). PCV = delivers set pressure with decelerating flow for set time. Volume is dependent variable (depends on driving pressure, compliance, etc.)

    Ventilation - pressure support

Also, pts get more flow w/ PSV vs VCV (flow-limited). PSV is more comfortable for awake ICU pts than VCV, I'm not sure if this translates into reduced GA requirements in OR. Probably not significant beyond academic interest.

    Separate consent for regional?

Our anesthesia consent form (PP in OK) has sections for general, neuraxial, peripheral regional, MAC, and IV regional (we do a lot of Bier blocks). Each section has subsections for expected result, technique and possible complications. In residency (in OH) we had no paper anesthesia...
