Personally I'd think externships and fresh LORs in both would be helpful and considering applying to friendly prelims as well. The scribe thing you are describing and working in a doctor's office sounds more like a premed type experience.

Well not all residencies are sponsored by universities or the connection could be tenuous at best. I can totally see a for profit hospital or even a nonprofit community hospital doing this.

UCSF came right out and announced on my interview day that they don't have good aid. The school has to have a great endowment to support this--Harvard, Penn, Columbia. Being a top school doesn't perfectly correlate with it.
