Adrenal incidentaloma - how much do y'all work it up in the hospital?

Question for hospitalists out there. CT imaging so often shows an adrenal incidentaloma. I see various practices regarding what is done, and I'm wondering what y'all's practices are. 1) Rule out hormonal activity in the hospital (aldosterone renin ratio, dexamethasone overnight suppression...

getting paid for nursing home patients?

Okay, so my friend told me that he knows primaries who are seeing their patients multiple times weekly and thinks that the 30-60 day thing is more likely a minimum requirement rather than an upper limit. Anyone who is primary who can comment on this?

getting paid for nursing home patients?

My friend works at a SNF as a consultant. He sees patients a couple times a week and bills for each visit with each patient as a separate outpatient encounter. My questions: -For those docs who do this as primary, why not see each patient multiple times weekly? You would be more familiar with...

    Rural - Specialist F/u vs PCP F/u

1 - Hey y'all, if you have an osteo or endocarditis patient, and you're a hospitalist in a rural area without ID around for >1 hour, do you end up sending them to follow up with a local PCP to monitor interval CRP/ESR (osteo)? Or do you make sure they see an ID doc even if the distance is...

    Just finished my em fellowship!

You mentioned an IM person in your cohort. Do you know if many of these FM —> ER fellowships are open to IM people? How many lines, etc, did you get to do in residency compared to the fellowship? If you are not going to work in a large ER but maybe want to cover the occasional shift in a...

    Practicing in the prison system?

"But I was told that if you work for any prison system, it's the "last job you will ever take in medicine", b/c you will be sued so many times that even IF you win all or a majority of the decisions (which you will... nobody sues the state/fed successfully for the most part), that other places...

How hard is a FM residency in Texas?

If you're looking at Texas, look into JPS in Fort Worth, Waco FM, and the program in Tyler, TX. All three are supposed to be pretty good, although if location is your biggest concern, JPS is pretty close to Dallas and you might like Ft. Worth by itself. Waco is about 1.5-2 hours from DFW. Up to you.

    Easiest Hospitalist Gigs You've Found

A lot of people talk about difficult hospitalist gigs, but what is the easiest job you've had or seen? You can define easy in your response, but specifically I'm thinking fewest patient encounters per day, no procedures, or you could say most time off without being too rough when you're on duty.

    MSK book recommendations

Lippincott primary care orthopedics is the 6th grade version of Essentials. Although Essentials has a lot of white space on the pages, it is still a far better value than the Lippincott book in terms of content, scope, and appropriate audience. The Lippincott book is shorter and unfortunately...

ICU book errors... does no one else notice this crap??

I just finished going back through the ICU book's acid-base chapter, and I just don't understand how these errors made it into a FOURTH edition text as famous as this one? This text is trying to convince me that the chronic compensation for a primary respiratory alkalosis (ph 7.54 and pCO2 of...

    Round and Go

    1. How common are the hospitalist jobs that allow one to round on patients in the morning, finish your notes, and then leave for the day? Are they more common at academic centers/larger hospitals? 2. Does it usually entail a pay cut? 3. Are there a lot of variations, such as taking call from...

Your opinion on applying to current AOA IM residencies (2018)

At this point, any program that has not already received preliminary accreditation (or whatever the technical term for "all your ducks are in a row at this stage of the game") is a danger to your future. This is the case because they may or may not have be accredited by the deadline. Why choose...

Advice for IM 2018-2019 Applicants Compilation

I had a pretty benign experience overall, but one person who applied radiology from my class was asked by a program director, "What can we do to make you come here?!?" My friend has great scores and so he ranked the program #2. When match day came, he got #8, so the moral of the story is 'don't...
