By the time you apply for fellowship, med school is a long way back at the end of your CV. I would say it is equally competitive, but you face a larger pool of "real" competitors. On second thought it may even a little more competitive if you don't develop in your career as much as your peers...

Hi! I just came to know that they will have a new fellowship option at my home program. One spot for 2 years Pulmonary only track starting next match season. We will have all options: 3 years PCCM combined, 2 years Critical Care and 2 years Pulm!

depends heavily in your immigration / visa status. Some programs will drop your file if you're not elegible for T32 funding. Some other will not sponsor H1B visas. Apply anywhere you'd be happy training. don't apply where you'd rather not go. The midwest has excellent schools that haven't had...
