    ASCO 2024

Let's not forget this funhouse mirror: https://meetings.asco.org/abstracts-presentations/234877 Phase III randomized trial of intensity-modulated proton therapy (IMPT) versus intensity-modulated photon therapy (IMRT) for the treatment of head and neck oropharyngeal carcinoma (OPC).

Big Rad Onc performing TH consults

Once you go through the process in a state, relatively easy to keep. My take is this lawsuit going to fail pretty quickly. What's stopping Dr. MGH or MSKCC from getting licensed in California or wherever? If they're so great, should be easy to get. Optics are poor as well, denigrating the...

Yeah, and one is even an "academic" institution.

Aware of some practices where no reimbursement = no wRVU.

Programs that have increased/decreased resident numbers and closed 2019 to 2024

This is a potential future for rad onc. ML/AI for contouring and plans. Physics still around for LINAC QA and software maintenance. RTTs still deliver. Surgeon or med onc approves contours/plan -- they are the responsible/liable MD. Rad onc fades away like nuc med.

    Rad Onc Twitter

Have suggested to my (excellent) dosimetrist(s) that are almost entirely remote that they show up once a month or so. A little harder to fire someone when you know their face.

    Rad Onc Twitter

There are no regulations of who can do dosimetry other than HR. Ultimately no one cares who did the plan, bc the Rad Onc is responsible. But don't begrudge the current slate of dosimetrists making as much bank as possible. The profession won't disappear but the current numbers will no longer...

Programs that have increased/decreased resident numbers and closed 2019 to 2024

Sure it's a bit glib, but only a start. The reality is the only way to close a program is to raise "standards" at the ACGME. It would be good to make a case of what changes are necessary to make a "good" training program. I would argue that a minimum of 2 residents a year is part of that...

Close every program with less than 8 positions. Slices off 209 positions.

    The end of TORS?

Pretty much. Title of this thread tongue in cheek. ORATOR2 will not convince any HN surgeon. The "my hands" argument is undefeated. But can say one thing for sure... if the curves were reversed, and RT was inferior... this field would never see another definitive RT patient again (unless...

    The end of TORS?

Yeah. 5 sec on clinicaltrials.gov to find an example; here from BMS with a search for nivolumab. 4th trial on list of 52 phase II trials. Most will have early stopping rules just like ORATOR2, which was planned to go to 140 total patients.

    The end of TORS?

Um what? I see a large number of these 30-75 patient phase II pharma trials. Pharma will seed a few varying designs or approaches in different regions/countries. The negative ones are never published or buried in some no name journal. The positive ones? Well they're in NEJM and Lancet these...

    The end of TORS?

It's not just the 2 "on treatment" deaths. Looking at the OS curve there are a few more at later timepoints in the TORS arm, as well as additional PFS events. ORATOR2 had their pre-specified stopping rules, just like most phase II trials. With a highly curable cancer, cannot justify such a...

    Non competes

The AHA isn't happy. Ultimately this will be tested in multiple lawsuits before it all shakes out.
