WAMC 518 c3.72/s3.81 School list help

Hey! I'm looking for some advice on my school list, especially with which T20s to apply to:
- First time applicant
- sGPA: 3.81
- cGPA; 3.72
- MCAT: 518 (131/126/132/129) (9 point increase from retake)
- ORM
- East Coast Resident
- Top 5 Public School Grad
- Clinical experience: 192 Volunteer EMS, 384 Paid EMS, 1280 Paid ED full time and counting
- Research experience and productivity = approximately 1000 hours, Fellowship to pursue research overseas over the summer, 1 poster. Recently started clinical research at local hospital and will be publishing on 4 projects over fall and spring of upcoming year.
- Shadowing: 150 hours (PC, Specialty Surgery, ED)
- Non-clinical volunteering: ~ 800 hours
- Leadership: VP at healthcare club for 4 years, raised $20k+ over 4 years
- Gap year and application year (2 total)
- Wedge score: 85.5

To note:
I was averaging 3.88 c/sGPA till my very last semester: normally my semester GPA were between 4.0 and 3.75, last semester was 2.8 (received B-,C+,C-,C+, A, A, 21 hours). Overloaded myself and fell behind and couldn't catch up plus busy with senior year capstone projects, etc basically just had a bad time.

Questions: Since my CARS is 126, which T20s are going to screen me out? Or do they not screen out 126s? On MSAR, many T20s have the 10th percentile at 127. Does it make sense to apply to these schools?

Soup kitchen volunteer, volunteering at food pantries, hospital volunteer, orgs that help homeless transition into homes and check in with them, church volunteering and camp counselor.

I'd like to be a physician (surgeon hopefully) who also does a lot of clinical research and teaching. I am looking for school with decent to great research opportunities and hope to get into academic medicine. I TA'd for 4 semesters and enjoy teaching and mentoring.
