Surgical subspecialists?

Psych?

Certainly not Gas?

Whos ACEP equivalent isn't actively trying to tank their respective specialty?

Is there a standard to aspire to?

Psych has their own APP problem, psych NPs are some of the highest paid APPs (besides CRNAs) and is one of the fastest growing APP fields. If they're not careful they'll find themselves in the same position as anesthesia.

Probably surgical sub-specialists, due to the relatively few physicians that are in each subspecialty (compared to EM, IM, Anesthesia, etc), and the length of training. I don't know a lot about derm, but from what I've heard talking to some friends in the field, derm is under attack from cosmetic clinics run by family/internal medicine doctors and APPs who advertise themselves as equivalent to dermatologists, and do acne scripts, injections, fillers, etc.

What I'm trying to say, is that unless you're willing to do 8+ years of miserable training and commit to a miserable surgical lifestyle, you're going to have to accept that you may have some midlevel encroachment, it's unfortunately a by-product of nursing administrators, the corporatization of medicine and the public's general distrust and dislike of physicians.

I'd settle for disbanding them altogether and starting from scratch. I'm not for unions, but I think we could put together a single organization that would represent EPs better.

You definitely need a street-fighter type who fights for the pit Docs, as opposed to one that services the CMG/Admin/Hospital-Insurance Complex. If such a person does come along and gets elected without the help of, and without being destroyed by the Machine, it would be a Black Swan even, for sure.

Maybe it's a perception plus regional thing, but I'm not seeing this as much as people talk about it on SDN. No doubt the respect or deference isn't what it used to be, but it's not like people throw tomatoes at me upon my arrival to bedside.

I think I started shadowing physicians about 10 years ago. Worked a job at the bottom of the clinical totem pole for 2 years before med school. Now about to graduate and match EM. Maybe I have been supremely lucky, but the only time I have been disrespected or seen physicians disrespected was by the true sociopathic malingers in psych holding. People of every stripe in the southeast seem to be generally grateful for the care they receive at all levels. It also helps to be a kind and approachable person as well. The only attending who has ever lamented to me the disrespect he receives from his patients was an obvious ass himself.

That has certainly not been my experience, but I hope it continues to be your experience as you start residency and move into your career.
