    The grass is always greener...

Just a quick note to help decrease some of the trolling we have seen roll through here on a slowly increasing basis: Threads of US vs THEM will be closely watched. Once they even begin to degenerate they will be cut off at the knees, buried in a shallow unmarked grave and lye spread all...

    Nurse Practitioner vs. PA

Enough is enough - I had been hoping the rather calmer folks suggestions would be taken in this thread but I'm tired of having to constantly check on it...thus it's being locked. Additionally, a few folks will be blocked from posting for a while due to some really un-kind personal attacks...

    Katrina Victims in Your ER

Had my first visit yesterday from a NO resident - he hadn't had his methadone for almost two weeks now and was NOT a happy camper - made worse by not writing for methadone out of the ED! Dennis

    Hostile takeover

Actually, while I fully agree with the original response I just need to add a slight clarification. Speaking for the PA side of the house, we can and do handle the same situations as the physicians do - it is dependent on the hospital where you are at. Many of us do run codes, handle...

    Hankypanky

While I'm not going to close this thread, it is one that will be watched closely. With the only two posts being from folks with no posting history and an initial post that can be consider quite insulting if not an attack I have my doubts that this will go anywhere good. However, it will be...

The site owner has placed a message indicating the site has been hacked and may be down for a limited time (perhaps a week)

    third degree/full thickness

Generally a true statement. It is very difficult to have a 'pure' full thickness burn in a patient. So while the core area may be painless the surrounding less than full-thickness burn will still be painfull. Dennis

    Drug seeking in the ED

While we have not eliminated Demerol from the formulary, it is probably something that will come down the road. Seekers are certainly one part of that decision, but the other part is that it is becoming more clear that demerol is not as safe as it has been thought to be. The metabolite from it...

    DPM discussion

For those that were asking about a DPM forum: After discussion again in the moderator world we've decided that based on previous attempts it is not the right time to try this again. We have twice in the past have dedicated DPM discussion threads and have had very very limited participation...

    ER Hall of Fame

Not to break into the tooth:tattoo ratio discussion, but I was very excited last night to see a young man with an ETOH of 625. He really did breath much better after being snorkled.....that's the high mark any of us had seen for ETOH. Dennis

    Allied Health Professionals?

Hey all - sorry for the slow reply to this issue but I've been at a CME conference....in short however, I'll bring the issue up with the big-dogs and will let you all know how the discussion goes... Dennis

    ER Hall of Fame

New personal record I saw last night - new onset DKA in a youngish guy - probably a simmering type 2 that got pushed over the edge. BS initially of 1965. Prime effort there! While you knew he was going to be high just by looking at him, he was carrying on a (generally) coherent...

    PA girls vs med girls

Clearly not relevent nor appropriate for this forum. If you choose to have such discussions, please head to the lounge. First and last warning. Dennis

    ABG question

DKA yesterday at 7.02 and a BS initially of 832...doing well today D

Question about analgesia in emergency patients

Fentanyl is a nice short-acting med also - if it is something that will need some observing or a delay before the OR / surgery eval, you can control pretty high levels of pain and yet have a short half-life if your surgery staff insist on no pain control before there exam. Dennis
