This really is an opinion thing. I wouldn't call UCSF or Duke or Wash U top tier based on reputation. I'd call them top tier based on rank. Most people haven't even heard of Wash U and regard Duke as a basketball school and don't know the difference between UCSF and any of the other UC's...
